Hi, I’m Hannah
The founder of To Be Connect, a social worker and teacher who supports older adults and their families with connection, companionship and guidance through the aged care system (support at home). I believe support should feel human, personal and grounded in genuine connection.
My story
I’ve always been drawn to hearing people’s unique life stories. Even as a child, I found comfort in creating safe, quiet spaces for myself by building cubbies, cosy corners and little worlds where things felt calm, grounded and connected.
That instinct followed me into adulthood.
Over the past decade, I’ve worked as both a Social Worker and a Primary School Teacher, in Australia and overseas. I’ve supported families through complex systems and worked closely with older adults navigating major life transitions.
Alongside my professional work, supporting my own grandparents through the aged care system showed me just how confusing and overwhelming this stage of life can be, even when families are trying their best. I saw how much pressure adult children carry, and how easily connection can be lost amongst paperwork, assessments and worry.
What I’ve learned, through my work and my life, is that the most meaningful support does not happen across a desk. It happens while walking together, making a cup of tea, folding laundry, sitting on the deck, or simply sharing space. These moments create safety, trust and genuine connection.
To Be Connect is the natural expression of everything I’ve always been drawn to: creating safe spaces, listening deeply, simplifying what feels overwhelming, and helping people feel less alone.
Why I Do This
Through my work in aged care, and being closely involved in my own grandparents’ ageing journey, I’ve seen how much mental and emotional load this stage of life can carry for families. Even when things are going well, the systems, decisions and ongoing coordination can quietly take up a lot of space.
I created To Be Connect to ease that load. To offer support that feels personal, consistent and genuinely human. By providing regular connection, gentle check-ins and guidance through aged care, families can feel reassured knowing their loved one is supported, while freeing up more time and energy to simply be family.
